The Colorado Department of Health Care Policy & Financing (the Department) contracted with Health Services Advisory Group, Inc. (HSAG) to administer and report the results of the Child and Adult Consumer Assessment of Healthcare Providers and Systems (CAHPS®) Patient-Centered Medical Home (PCMH) Surveys.1-1 The goal of the PCMH Survey is to provide performance feedback that is actionable and will aid in improving overall patient-centered satisfaction by administering the survey at the practice level. The implementation of the CAHPS PCMH Surveys was a pilot study and the results presented in this report are baseline results.
