Effective March 1st 2012, medical coverage will expand through the WAwD to include individuals who are at least age 16 but less than 65 years of age, who are employed, have been determined disabled and meet all other eligibility criteria. The Healthy Communities program is charged with reaching out to families with children, and pregnant women newly enrolled in Medicaid or CHP+; to explain their benefits and help them find a medical home/primary care provider. During this initial orientation they also discuss the importance of well-child visits with families, and then follow up over time with reminders when it is time for a well-child visit.
